Rotary Club: Anchorage South Project Title: Forget Me Not Chorus 1. Describe the project. What was done, when and where did project activities take place? The Forget Me Not Chorus is a community based initiative that enriches the lives of Alaskans affected by Alzheimer's Disease and related dementias by providing "Music Therapy" that has a profound therapeutic impact on individuals with dementia. This is an 8-week program that that brings together individuals with Alzheimer's or related dementias, caregivers and community volunteers to practice singing sessions which culminated in a Fall (2025) Public Concert featuring, songs, and personal stories to foster connection, dignity and joy. 2. How many people benefited from this project? 100 plus 3. Who were the beneficiaries, how were they impacted by this project, and what humanitarian need was met? The individuals with Alzheimers or dementia as well as the caregivers, and community volunteers. Anchorage South Rotary had a club member (Yaghen Ataian) who worked one on one over the 8 week period with an Alzheimer patient to prepare for the concert. On the day of the concert 6 Anchorage South Rotarians ushering and helping for the event. (Kathy Grimes, Timothy Kane, Kim Hays, Vesta Herrick Amy Mackey Hornak & Yaghen Ataian. 4. How many Rotarians participated in the project? 6 5. What did they do? Please give at least two examples, not including financial support provided to the project. Yaghen Ataian - Worked with the Alzheimer Patient to prepare for the concert over an 8 week period. 5 Other Anchorage South Rotarian helped on the day of concert, ushering is guests, handing out snacks, set up and clean up as needed. 6.
