The HOME-BASED PALLIATIVE CARE PROJECT aims to provide comprehensive, community-based, home-centered palliative care services that ensure dignity, comfort, and psychosocial well-being of the patients and their families living in the district of Hassan of Karnataka state in India. The project caters to the patients suffering from longstanding life limiting or life altering illnesses like Cancer, stroke, Parkinson disease, cardiovascular diseases, dementia, gangrene, spinal disorders, renal failure, motor neuron disease, HIV/AIDS, and other debilitating conditions. The project addresses the emotional, psychosocial, and spiritual dimensions of care for all patients served, thereby improving the quality of life of individuals with the aforementioned long-standing diseases, by providing need-based medical and non-medical services at both care centers and in patients homes.

Controlled drugs will be prescribed by Dr. Deepak P (MBBS and MD in medical pharmacology, working as the Head of the department of Pharmacology in the local Government Medical College in Hassan) after examining the patient and assessing the requirements of these medications. He has cleared a certificate course in Essentials of Palliative care conducted by Indian Association of Palliative Care, a copy of which has been attached.

The project is designed to create a collaborative platform involving Government, academia, NGOs and the community to strengthen and promote the importance of palliative care in Hassan District thus ensuring sustainability even after the project period ends.

Objectives: Identify patients who need palliative care, provide appropriate services, and improve quality of life for patients and their families.
